At 51, my body went sideways on me. Not all at once. First the 3am wake-ups — wide awake, heart going, for no reason at all. Then the belly. Hard and high, like a shelf under my ribs. Flat at 7am. Six months pregnant by dinner.

I wasn't eating more. I was eating less. I walked every morning. I cut bread, then wine, then dessert. I did 1,200 calories for six weeks and lost three pounds and every bit of my patience.

The question nobody asked me

Every doctor asked what I ate. How much I moved. Whether I'd tried cutting carbs. Not one of them asked the question that mattered:

What time of day does your belly get worse?

Because for me, the answer was obvious once I looked. Mornings were fine. By 4pm, my waistband was digging in. By evening, I was puffy everywhere — belly, face, fingers. And by 3am, I was awake.

What your doctor won't tell you about cortisol

Cortisol is your stress hormone. In a healthy rhythm, it peaks in the morning to get you going and drops at night so you can sleep.

For most of your life, estrogen helps keep that rhythm in check. Then perimenopause arrives, estrogen drops — and for many women, cortisol stops coming down at night.

When cortisol stays high in the evening, three things happen at once:

1. Your body stores more fat around the middle. Belly fat cells are especially sensitive to cortisol. It's why the weight lands on your stomach and not your arms.
2. You hold on to water. That's the evening puffiness — belly, face, rings that won't come off.
3. Your sleep breaks around 3am. Your body fires a stress signal when it should be resting. You wake up wired.

Why everything you tried made it worse

Once I understood this, I finally understood why I'd been failing for two years. Most of what women are told to do after 45 raises cortisol even more.

Strict dieting

Your body reads hunger as danger. A 2010 study in Psychosomatic Medicine found that restricting calories raised women's cortisol. A strict diet against a cortisol belly is gasoline on a fire.

Hard workouts, fasted

Intense exercise on an empty stomach is another stress signal. For a body already stuck "on", more stress is the last thing it needs.
